Kuali Protocols and Kuali COI notifications- Sent Sunday

Some of you may have been notified recently regarding notifications sent out Sunday. We just wanted to make you aware of an issue Kuali encountered while performing an upgrade to their core databases during the maintenance window this past Sunday. This issue caused some valid protocol and COI notifications to send in duplicate to recipients.  During the upgrade two of Kuali's notification servers were inappropriately active at the same time which caused duplicate notifications to send.  This was discovered and resolved earlier Monday and  a retro on this issue will be done to make sure Kuali takes necessary steps to avoid this in the future.  We apologize for the inconvenience and any confusion the duplicate emails sent earlier today caused.  If you have any further questions please let your compliance officer know, if they have not reached out to you already. These emails would have been sent during the window of 11:38am - 2:54pm PT (the bulk were sent between 11:38am - 11:50am PT) Sunday.  Again, we sincerely apologize for the inconvenience and confusion this caused our users.
