Updated F&A Waiver Form

OSP's updated F&A waiver form can be accessed from our forms page and is attached here for reference. This new version requests information regarding the sponsor, provides additional space to complete section A, and clarifies instances when a sponsor's written policy may be evaluated on a case by case basis.
